Your Office Visit

Preparing for Your Appointment

Before arriving for your appointment, please make sure that you have the following items with you:

  • Your insurance card
  • A photo ID
  • List of all current medications
  • Report and film/CD of any MRI or CT
  • Any X-rays that may have been taken of your injury or condition. Our physicians may not be able to see you without your X-rays.
  • If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit. Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit

Upon your arrival, a clinic services representative will make copies of your insurance card and ID. They will verify that all information in our database is accurate. They will then provide you with an IPAD to complete necessary forms. Lastly, they will collect your copayment.
